Arthritis Management: Living Well with Joint Pain

Arthritis โ€” literally meaning "joint inflammation" โ€” is not a single disease but a term covering over 100 conditions affecting joints, surrounding tissues, and connective tissue. In Botswana, osteoarthritis (OA) of the hip and knee and rheumatoid arthritis (RA) are among the most common conditions we manage at Healing Hands.

The most important message for anyone living with arthritis: movement is medicine. The instinct to rest painful joints is understandable โ€” but research consistently shows that the right kind of exercise reduces pain, improves function, and protects joints from further damage.

Understanding Your Type of Arthritis

Osteoarthritis is the 'wear and tear' arthritis, characterised by degeneration of joint cartilage. It most commonly affects the knees, hips, hands, and spine. Risk factors include age, obesity, previous joint injury, and family history. Rheumatoid arthritis is an autoimmune disease in which the immune system attacks the joint lining, causing inflammation, pain, and eventual joint damage. It requires medical management with disease-modifying drugs in addition to physiotherapy. Physiotherapy plays an important role in both conditions, though the approach differs.

Why Exercise is Not Optional

Joint cartilage has no direct blood supply โ€” it receives oxygen and nutrients from the fluid that circulates through the joint during movement. Immobility literally starves the joint of nutrients. Exercise also strengthens the muscles that protect and stabilise joints, reducing the load placed on cartilage during daily activities. Clinical guidelines from the American College of Rheumatology, Arthritis Foundation, and European League Against Rheumatism all recommend exercise as a first-line treatment for osteoarthritis and rheumatoid arthritis.

Best Exercise Types for Arthritis

Low-impact aerobic exercise such as swimming, water aerobics, cycling, and walking are excellent for arthritis as they improve cardiovascular health without excessive joint stress. Strength training is critical โ€” particularly for osteoarthritis, where quadriceps weakness is a major risk factor for knee OA progression. Range-of-motion exercises maintain joint flexibility and reduce stiffness. Balance training reduces fall risk, which is elevated in people with arthritis.

Quality sleep is essential โ€” poor sleep dramatically worsens pain perception. Stress management through mindfulness or relaxation techniques addresses the psychosocial dimension of chronic pain.

Red Flags: When to Seek Urgent Care

See your doctor urgently if you experience sudden, severe joint swelling with redness and warmth (possible infection or crystal arthritis); a joint that is locked and cannot be moved; fever alongside joint symptoms; or rapid worsening of symptoms over days. These require medical assessment before physiotherapy.
